This type of assessment is based on contacts / cases in other forms within bxp software. It is possible to link an assessment to a record (CDA) or to a specific contact (CCL).
The question "Load from: Form" allows the user to select from which form the CDA records / CCLs contacts will be loaded.
The question "Form Load: Use" allows the user to select from which form the CDA / CCLs will be loadedassess records or contacts.
